Use of an family planning method during the postpartum period is important both to space births, and to prevent unwanted pregnancies. Under-utilization of family planning services during this period not only leads to adverse conditions caused by excessive fertility, but also has negative effects on both maternal and infant health. Women are in close contact with health care providers before, during, and after childbirth, when they are most likely to be impressed by the services they receive. Effective provision of health care services in these periods is therefore of great importance. Literature indicates high rates of success and compliance with family planning services provided during these periods. Many women neglect to use a modern contraceptive method in the postpartum period believing that breastfeeding alone ensures sufficient protection against pregnancy. Indeed, breastfeeding can be 98% effective as a contraceptive method; however, this is only true if certain criteria are observed. There are three basic criteria for the lactational amenorrhea method to be effective: the baby must be less than six months old, must be exclusively or almost exclusively breast-fed, and the mother must not be menstruating. In a conference held in Bellago in August 1988 by the World Health Organization and other international agencies, a consensus was developed for effective use of lactational amenorrhea method, and it was agreed to adopt breastfeeding as a potential family planning method in maternal and child health programs. Doğal Bir Aile Planlaması Yöntemi: Laktasyonel Amenore Yöntemi Özet
Doğum sonrası dönemde aile planlaması yöntemi kullanma, hem doğum aralıklarını açmak, hem de istenmeyen gebelikleri önlemek açısından önemlidir. Bu dönemde AP hizmetlerinden yeterince faydalanılamaması, aşırı doğurganlığın getirdiği olumsuzlukların yanı sıra, doğum aralıklarının kısa olması nedeniyle hem annenin hem de bebeğin sağlığı olumsuz yönde etkilenmektedir. Doğum öncesi, doğum ve doğum sonrası dönem, kadınların sağlık personeli ile yakın ilişkide oldukları ve verilen hizmetten en çok etkilendikleri dönemlerdir. Bu nedenle bu dönemlerde sağlık bakım hizmetlerinin etkili sunumu önem taşımaktadır. Literatürde bu dönemde verilen aile planlaması hizmetlerinde başarı ve yönteme uyumun yüksek olduğu belirtilmektedir. Doğum sonrası dönemde kadınların etkili aile planlaması yöntemi kullanmalarını etkileyen pek çok faktör vardır. Bu faktörlerden bir tanesi emzirme davranışıdır. Doğum sonrası dönemde kadınların modern aile planlaması yöntemi seçmelerini olumsuz etkileyen emzirme belli kriterlere uyulduğunda %98 gebelikten koruyucudur. Doğal aile planlaması yöntemleri arasında yer alan laktasyonel amenore yönteminin etkili olabilmesi için; bebeğin altı aydan küçük olması, bebeğin tam veya tama yakın emzirilmesi ve kadının menstrüasyon görmemesi olmak üzere üç temel kriter vardır. Dünya Sağlık Örgütü ve uluslararası kuruluşlar tarafından Ağustos 1988’de Bellogo’da yapılan bir konferansta, laktasyonel amenore yönteminin etkili olabilmesi için uyulması gereken kurallar çerçevesinde prosedür oluşturulmuş, emzirmenin anne ve çocuk sağlığı programlarında potansiyel bir aile planlaması yöntemi olarak kabul edilmesine karar verilmiştir.
