AIM: Keeping health records regularly is important for determining the health status of the population and planning health services. It is observed that the existing health record systems in Istanbul are insufficient in determining the number of pregnant women, puerperants, newborns and preschool children.
METHODS: Therefore, an intervention study was planned by Istanbul Directorate of Health in order to provide systematic monitoring of pregnant women, puerperants, newborns and preschool children, and ensure that equity in the distribution of qualified prenatal and postnatal health care is maintained. This project called GEBLİZ has been started to be used since September 2008.
RESULTS: According to this project all public and private health centers in which pregnant and child care are given have the responsibility to transfer necessary information about the patients to an electronic database. Through this computerized system, a connection between primary, secondary and tertiary health care settings has been maintained, and deficiencies of paper records have been completed. Health records have become more consistent.
CONLUSION: Compared to one year before the start of the intervention, there have been important increases in the number of pregnant women, puerperants, newborns and infants detected by primary health care units. Besides, “home visits” which have been neglected were started actively by primary health care personnel, and preventive health service who stayed in the shadow of therapeutic health services came to life again.

İstanbul’da Gebe, Lohusa, Bebek ve Çocuk İzlemlerinin İyileştirilmesi: GEBLİZ Özet
GİRİŞ: Sağlık kayıtlarının düzenli olarak tutulması, toplumun sağlık düzeyinin saptanması ve gerekli hizmetlerin planlanabilmesi için oldukça önemlidir. İstanbul’da mevcut sağlık kayıt sistemlerinin, sağlık hizmeti sunumunda öncelikli yere sahip olan gebe, lohusa, bebek ve çocukları tespit etme ve takiplerinin yapılmasında yetersiz kaldığı gözlenmiştir.
YÖNTEM: Bu nedenle İstanbul’daki gebe, lohusa bebek ve çocuk sayılarının tam olarak tespit edilebilmesi, tüm anne ve bebeklerin yeterli sayıda ve kaliteli doğum öncesi ve sonrası sağlık hizmeti alabilmesi için İstanbul İl Sağlık Müdürlüğü tarafından bir müdahale çalışması planlanmış ve “Gebe, Lohusa, Bebek, Çocuk İzlem Sistemi (GEBLİZ)” olarak adlandırılan proje Eylül 2008 itibarı ile hayata geçirilmiştir.
BULGULAR: Gebe, lohusa, bebek ve çocuk izlemi yapılan tüm kamu ve özel sağlık kuruluşlarının verilerini elektronik ortama aktarması temeline dayanan bu sistem ile birinci, ikinci ve üçüncü basamak, kamu ve özel sağlık kuruluşları arasında düzenli bir iletişim ağı oluşturulmuş, elektronik kayıtlar ve veri giriş formlarının birlikte kullanımı sayesinde “Form 023 Birinci Basamak Sağlık Kurumu Aylık Çalışma Bildirisi” başta olmak üzere veri giriş formlarındaki eksik veri girişleri tamamlanmış, veri tutarlılığı sağlanmıştır.
SONUÇ: Projenin bir yıl öncesi, sonrası ile karşılaştırıldığında bir yıl içinde birinci basamak sağlık kuruluşlarında gebe, lohusa, bebek ve çocuk tespit ve izlem sayılarında önemli artışlar olduğu gözlenmiştir. Bunun yanında İstanbul’da unutulmaya yüz tutmuş olan “ev ziyaretleri” birinci basamak sağlık personeli tarafından aktif olarak yapılmaya başlanmış, tedavi edici sağlık hizmetlerinin gölgesinde kalmış olan koruyucu sağlık hizmetlerinin tekrar canlanması sağlanmıştır.
