Evaluation of the vitality levels of the teeth in the distracted segment in procedures of premaxillary distraction osteogenesis

A B S T R A C T 

Abstract
The purpose of this study was to evaluate the losses in pulp vitalites occurring in teeth adjacent to the mobilized segment following premaxillary distraction osteogenesis. Twelve patients (4 males and 8 females) constituted the study group. Electrical pulp test was used to assess the vitality of the upper first bicuspids, canines and lateral and central incisors adjacent to the mobilized segment. Tests were quantitatively performed preoperatively, and on the 5th, 7th, 10th, 30th and 60th postoperative days. The affection and healing processes of the 24 upper central and lateral incisors examined were in parallel with each other. The affection and healing processes of the 12 canines which were vital preoperatively and included in the statistical analysis were significantly different from the other teeth when the vitality levels of the canines and small first bicuspids adjacent to the osteotomy line were assessed. However, a positive response was achieved in all the teeth on the 60th postoperative day. No sensorial loss was detected in small bicuspids which were adjacent to osteotomy line, and however, not present in the segment. In order to preserve the pulpal blood during osteotomies of the jaws, a safety margin of 5 mm is recommended between the apices of the teeth and the osteotomy line. Premaxillary distraction osteogenesis is a safe procedure regarding the pulpal changes of the teeth.

Premaksiller distraksiyon osteogenezi uygulamalarında distrakte edilen segment içerisindeki dişlerin vitalite düzeylerinin incelenmesi

Özet
Bu çalışmanın amacı, premaksiller distraksiyon osteogenezi sonrasında segmentle ilişkideki dişlerde oluşan vitalite kayıplarının incelenmesidir. Çalışma grubunu 12 hasta (4 erkek and 8 kadın) oluşturmuştur. Hareketlendirilen segment ile ilişkideki birinci küçük azı, köpek dişi ve lateral ve santral keser dişlerin canlılık düzeyleri, elektrikli pulpa testi ile değerlendirilmiştir. Testler ameliyat öncesi ve postoperatif 5, 7, 10, 30 ve 60. günlerde kantitatif olarak değerlendirilecek şekilde uygulanmıştır. İncelenen 24 üst birinci ve ikinci keser dişlerin operasyondan etkilenme ve iyileşme süreçleri birbirleri ile paralellik göstermektedir. Osteotomi hattına komşu dişler olan köpek ve birinci küçük azı dişlerinin canlılık düzeyleri incelendiğinde ise, preoperatif olarak vital olduğu tespit edilen ve istatistiksel analize dahil edilen 12 köpek dişinin etkilenme oranları ve iyileşme süreçleri, diğer dişlerden anlamlı bir şekilde farklılık göstermiştir. Ancak 60. günde tüm dişlerde testlere pozitif yanıt alınmıştır. Segment içerisinde yer almayan ancak osteotomi hattına komşu küçük azı dişlerinde ise herhangi bir duyusal kayıp saptanmamıştır. Çenelerin osteotomileri esnasında dişlere ait pulpal kan akımının korunması amacıyla dişlerin kökleri ve osteotomi hattı arasında 5 mm’lik güvenlik sınırı bırakılması önerilmektedir. Premaksiller distraksiyon osteogenezi dişlere ait pulpal değişiklikler göz önüne alındığında, güvenli uygulamalardır.
