Musculo-fascial restoration with component separation and fascial release in a case of large abdominal defect and open abdomen

Abstract
Open abdomen and delayed abdominal closure is occasionally required when conditions of the abdominal wall or peritoneal cavity prevent closure. Open abdomen and extensive abdominal defect are difficult problems in reconstructive surgery. In this report a 45 years old male patient who had decompressive laparotomy due to intra-abdominal infection and compartment syndrome was presented. During the open abdomen follow-up, abdominal skin and fascial necrotizing infection of abdominal wall occurred and extensive abdominal wall defect developed. For the abdominal wall restoration; component separation, fascial-release procedures were combined and abdominal skin defect was treated with lateral abdominoplasty technique.

Geniş abdominal defektli açık abdomen olgusunda fasyal gevşetme (komponent separasyon) ve serbestleştirme ile muskulo-fasyal restorasyon

Özet
Açık abdomen ve geç abdominal onarım, abdominal duvarın ve peritoneal kavitenin kapatılamadığı şartlarda zaman zaman gerekli olabilmektedir. Rekonstrüktif cerrahide açık abdomen ve ileri derece abdominal defektler tedavisi güç problemlerdir. Sunulan olgu abdominal enfeksiyon ve kompartman sendromuna bağlı dekompressif laparatomi geçirmiştir. Hastanın açık abdomenle takibi sırasında, nekrotizan enfeksiyona bağlı abdomen cildi ve fasyal dokularında geniş defektler meydana gelmiştir. Hastada abdominal duvarın onarımı amacıyla; komponent ayrıştırma, fasyal gevşetme teknikleri kombine edilen hastada cilt defektleri için lateral abdominoplasti uygulandı.
