Carbamazepine intoxication induced prolonged thrombocytopenia: a case report

Abstract
Drug poisonings are important cases of emergency services that are common usually require a serious approach and respond well to treatment. The neurological and cardiac side effects may be seen in acute and chronic carbamazepine intoxications. Side effects should be controlled as soon as possible. Thrombocytopenia is one of the complications of carbamazepine intoxications. Low thrombocyte counts has no need of treatment, however, treatment is required if there is a bleeding and/or very low counts. Treatment approaches are repeated doses of activated charcoal and early hemoperfusion in carbamazepine intoxications. We are presenting in this case, the complication of long-lasting thrombocytopenia in a 23-years-old male patient with high dose intoxication of carbamazepine.

Karbamazepin intoksikayonuna bağlı uzamış trombositopeni: Olgu sunumu

Özet
İlaç zehirlenmeleri, acil servislerin sık karşılaşılan, ciddi yaklaşım gerektiren ve genellikle tedaviye iyi yanıt veren önemli olgularıdır. Akut ve kronik karbamazepin (KBZ) zehirlenmelerinde nörolojik, kardiyak ve hematolojik yan etkiler görülebilir. Yan etkiler kısa sürede kontrol altına alınmalıdır. KBZ’nin hematolojik yan etkilerinden biri de trombositopenidir. Düşük trombosit sayımları genel olarak tedavi gerektirmez, ancak, kanamanın ve/veya çok düşük sayımın olduğu durumlarda tedavi gerekebilir. KBZ zehirlenmelerinde genel olarak tedavi yaklaşımları, tekrarlayan dozlarda aktif kömür verilmesi ve erken dönemde hemoperfüzyondur. Biz bu olguda intihar amacıyla yüksek doz karbamazepin alan 23 yaşındaki bir erkek hastada uzun süre devam eden trombositopeni komplikasyonunu sunmayı amaçladık.
